Fadhli Azhari 3996e2f199 feat: initial Ollama MCP server 
TypeScript MCP server wrapping the Ollama REST API. Provides tools for:
- Text generation and multi-turn chat
- Model management (list, show, pull, delete)
- Health check and running model status
- Embeddings generation
